premier anti-aging life center). MAJOR RESPONSIBILITIES/ESSENTIAL
FUNCTIONS The Stroke Program Coordinator is a health care
professional who works in collaboration with the medical and
hospital staff to provide multidisciplinary care to the stroke
patient. The Coordinator will coordinate the efforts between
physicians, hospital administration, and staff while tracking the
progress of the program. The Coordinator is responsible for
coordinating and leading all activities associated with the Stroke
certification programs. The Coordinator provides clinical
professional expertise for patients, families and staff members.
Scope of practice includes providing organized care to the patient
with stroke, focus on health promotion and disease prevention. The
Coordinator also functions as an educator and coordinates care in
critical care/stroke patient care management, ensuring optimal
patient care management and outcomes. The Coordinator will collect
and analyze data surrounding the stroke patients to assist with
documentation, coding, clinical care and outcomes. Facilitates
development and refines protocol to become the standard of care
within the facility, while providing and participating in quality
improvement process. Applies excellent verbal and nonverbal skills
to achieve excellence in patient experience. Collaborates with all
members of the interdisciplinary team to achieve productive
movement of the best practices within the organization. Assists
with the development and implementation of initiatives, policies
and procedures, including staff education activities related to
stroke and cardiac care. Evaluates the programs through the use of
data capture, follow-up and trend analysis and presents findings to
appropriate committees. Collaborates with multidisciplinary team on
problem identification and resolution, cost containment issues,
implementation of new services and systems/performance measures for
the Stroke Program. Acting EMS liaison for Stroke Care. JOB
QUALIFICATIONS Minimum Education (Indicate minimum education or
degree required.) Bachelor of Science in Nursing Knowledge on
current State Laws and Regulations as applicable to facility and
Stroke Programs. Minimum of 8 hours annually of stroke
education/training through American Stroke Association, American
Heart Association, Society of Chest Pain Center, or any other
organizations offering stroke education/training (to be completed
annually by/with performance evaluation). Preferred Education
(Indicate preferred education or degree required.) Master’s Degree
in Science of Nursing Minimum Work Experience and Qualifications
(Indicate minimum years of job experience, skills or abilities
required for the job.) 3-5 years clinical and/or management
experience in a multidisciplinary health care setting, including
critical care which includes a minimum of two years of inpatient
experience. Must have analytical skills for collecting and
analyzing data. Ability to communicate effectively verbally and in
writing. Must be able to work in a union environment. Preferred

participate in quality improvement activities. Reviews EMR for real
time extraction of process analysis and identification of
improvement opportunities. Identifies, collects, enters, monitors
and evaluates indicator that reflect the important aspects of care
and service. Develops and collaborates with other members of the
healthcare team to implement changes to improve outcomes. Serves as
a clinical expert and resource, assisting in the care for complex
and/or highly specialized patient care situations. Coordinates the
multidisciplinary committee from across the continuum; makes
recommendations for process improvements with clinical processes.
Analyzes data and prepares reports; assists leadership in setting
goals, identifying roadblocks, and in establishing milestones for
project success. Responds when appropriate to Code Stroke calls for
real time quality improvement observations and interventions.
Initiates and participates in case reviews for Stroke patients.
Develops this new professional role. Remains current in literature
and best practice. Provides leadership for the Stroke program.
Collaborates with hospital educators to develop and implement
Stroke focused education. Ensures evidence-based protocols and
therapies are the standard of care followed within the facility.
Collaborates with medical and clinical staff to provide
multidisciplinary care to Stroke patients. Collaborates with
Quality Management to communicate real time feedback to clinicians
regarding missing bundle elements and required documentation.
Collaborates with the nursing team to develop and implement plans
of care including individualized teaching plans for patients and
families. Analyzes data trends to assist with documentation,
coding, clinical care, and improved outcomes. Participates in
rounding on patients with identified Stroke and proactively
responds to patient needs to improve patient outcomes and
positively impact overall patient experience. Knowledge in the care
of stroke and cardiac patients using the American Stroke
Association Guidelines All other duties as assigned Required
Licensure, Certification, Registration or Designation (List any
licensure or certification required and specify name of agency.)
Valid California RN License or Nurse Practitioner in the State of
California Basic Life Support (BLS) form the American Heart
Association (AHA) Advances Cardiovascular Life Support (ACLS) from
the American Heart Association (AHA) NIH Stroke Scale (NIHSS)
Certification Current Los Angeles County Fire Card (required within
30 days of employment) Assault Response Competency (ARC) (required
